
NEIGHBOURHOOD HEALTH & WELLBEING OUTREACH
The AT Beacon Project engages and empowers vulnerable groups to lead healthier lives. We work alongside communities, organisations and faith leaders to establish Beacon outreach sites in safe spaces embedded within local neighbourhoods
Our outreach sites provide drop-in sessions close to where people live or work, giving communities access to trusted information and support on matters concerning their health and wellbeing.
Beacon outreach sites support local communities to live healthier lives. Whether it’s blood pressure checks and health assessments, signposting into primary care and local support services, building vaccine confidence, or promoting mental health and resilience, we work with people to provide trusted information and support that helps them stay well. For those at higher risk, we also offer more structured support, including a Fruit and Vegetables on Prescription programme and Reward Card schemes to help people manage their health over time.
Our drop-in health and wellbeing sessions are led by a clinician alongside our Neighbourhood Ambassadors, trained peer educators from the local community.
No Beacon outreach site is the same, as we aim to deliver the most suitable approach for that locality.
